D-39 | Inpatient Outcomes of Mitral Valve Transcatheter Edge-to-Edge Repair (TEER) in Patients With Acute Decompensated Heart Failure

چکیده
TEER is a favorable alternative to surgery in patients with heart failure (HF) and mitral regurgitation (MR), but its effect acute decompensated (ADHF) not well studied. We aim study the short-term outcomes of these using largest US inpatient database. queried 2016-2019 NIS database ICD-10 codes identify all adult MR ADHF. Outcomes who underwent during index hospitalization were compared those did not. Multivariate logistic regression was used compare rates cardiogenic shock, mechanical ventilation, use circulatory support (MCS), cardiac transplant, kidney injury (AKI) need dialysis; linear length stay (LOS). Of 1,854,806 ADHF, 24,905 (1.36%) TEER. Overall mortality 3.39% significantly lower recipients [aOR 0.39 (0.30-0.49), p<0.01]. LOS shorter group (4 vs 5.9 days). had ventilation 0.81 (0.70-0.94), p=0.01] AKI 0.43 (0.39-0.47), p<0.01], although requiring dialysis comparable. higher shock. Rates MCS comparable except intraaortic balloon pump placement, which more frequent group. ADHF associated LOS. Prospective trials are needed establish as feasible intervention

Endovascular Edge-to-Edge Mitral Valve Repair
Background—The edge-to-edge technique is an accepted method for the surgical repair of a regurgitant mitral valve. This study reports the initial use of an endovascular technology that enables a double-orifice edge-to-edge mitral valve repair without cardiopulmonary bypass in an animal model. Methods and Results—Adult pigs (n 14) were anesthetized, and left thoracotomy was performed for epicard...
متن کاملAcute Mitral Regurgitation Secondary to Papillary Muscle Tear: Is Transcatheter Edge-to-Edge Mitral Valve Repair a New Paradigm?
Severe mitral regurgitation (MR) secondary to papillary muscle tear is an infrequent but overwhelmingly morbid and often fatal complication of acute myocardial infarction occurring in 1% to 3% of patients. In-hospital mortality without surgical correction can approach 80%; however, even with surgical correction, papillary muscle tear carries a mortality rate between 19% and 53%. While considera...
متن کاملpercutaneous mitral valve repair with the edge-to-edge technique: case series of first iranian experience
mitral regurgitation (mr) is a common valvular lesion in the general population with considerable impact on mortality and morbidity. the mitraclip system (abbot laboratories, abbot park, il, usa) is a novel percutaneous approach for treating mr which involves mechanical edge-to-edge coaptation of the mitral leaflets. we present our initial experience with the mitraclip system in 5 patients. in ...
متن کاملEdge-to-edge mitral valve repair without ring annuloplasty for acute ischemic mitral regurgitation.
BACKGROUND Alfieri edge-to-edge mitral repair has been used clinically with ring annuloplasty to correct ischemic mitral regurgitation (IMR), but its efficacy without concomitant ring annuloplasty has not been described in this setting. METHODS Seventeen sheep underwent implantation of 9 radiopaque markers on the left ventricle, 8 on the mitral annulus (MA), 1 on each papillary muscle (PM) ti...
متن کاملPulmonary Hemorrhage following Edge-to-Edge Mitral Valve Repair
Mitral valve repair with the MitraClip device has emerged as an effective treatment option for patients with severe mitral regurgitation and contraindications for surgical interventions. While the procedure is not known to cause pulmonary complications, we describe two cases of pulmonary hemorrhage following percutaneous mitral valve repair. The patients did well with supportive care and reinit...
